It seems pretty obvious what happened, but I bet the "Investigation" takes months and costs loadsamoney.

There was a similar runaway of a remotely controlled cement train in Tasmania three years ago which the ATSB (Australian Transport Safety Bureau) is STILL INVESTIGATING.
 Sir Humphrey Appleby would be proud of them ...............

Dreadful pollution in Picton Harbour. Engine was black & white when it went in but yellow and red when it came out. Swimming ought to be banned.
